Surgical Tooth Extractions in Brooklyn, NY

Removal of teeth that are broken at the gumline, impacted or have complex roots.

What Is a Surgical Tooth Extraction?

A surgical extraction is performed when a tooth cannot be removed using a simple approach. This includes teeth that have broken off at or below the gumline, teeth with curved or fragile roots, teeth fused to the surrounding bone, or teeth that are partially impacted. The procedure involves making a small incision in the gum tissue and, in some cases, removing a small amount of bone to access and remove the tooth safely. Surgical extractions require a higher level of clinical skill and planning than simple extractions but are routine procedures at Bensonhurst Dental Care.

When a Surgical Extraction Is Needed

A surgical extraction is needed when a tooth cannot be gripped or lifted out whole. A small opening in the gum, and sometimes dividing the tooth, allows it to be removed with less force and less damage to the bone.

  • A tooth broken off at or below the gumline
  • Curved, long or fragile roots
  • An impacted tooth
  • A root left behind by an earlier extraction
  • A tooth fused to the bone

Surgical Tooth Extractions at Bensonhurst Dental Care

The dental team approaches surgical extractions with careful planning, using digital imaging to evaluate the tooth’s position, root anatomy, and relationship to surrounding structures such as nerves and sinuses. This preparation ensures the procedure is performed efficiently and with the least amount of tissue disruption possible. Local anesthesia is used to keep the patient completely comfortable throughout, and sedation options may be available for patients who experience anxiety about the procedure.

What to Expect, Step by Step

  1. Imaging. X-rays or 3D imaging show roots, nerves and sinuses.
  2. Numbing. Local anesthesia is given, with sedation where appropriate.
  3. Access. A small opening is made in the gum.
  4. Removal. Bone is trimmed if necessary and the tooth is sectioned and removed in pieces.
  5. Socket care. The socket is cleaned and grafted if planned.
  6. Stitches. The gum is closed.

A surgical extraction is the gentler choice for a difficult tooth. Taking it out in sections is kinder to the bone than forcing it out whole.

Recovery and Aftercare

After surgery

The first 24 hours

  • Bite on gauze; do not rinse or spit
  • Use cold packs for the first day
  • Keep your head raised and rest

The following days and weeks

  • Begin warm salt-water rinses on day two
  • Keep to soft foods for several days
  • Return for stitch removal if the stitches are not dissolvable

When to Call Us

Call the office on (718) 234-3434 if you notice any of the following:

  • Bleeding that will not stop
  • Numbness of the lip, chin or tongue that persists the next day
  • Increasing pain after day three, swelling or fever

Healing and Replacement

After a surgical extraction, the site is sutured to promote proper healing. Patients receive detailed aftercare instructions covering pain management, dietary modifications, and activity restrictions during the recovery period. As with simple extractions, the dental team discusses replacement options and may recommend ridge preservation at the time of surgery to maintain bone volume for a future implant or other restoration.

Cost and Insurance

The fee for an extraction depends on how difficult the tooth is to remove. You are told the fee before the procedure.

What affects the cost

  • Whether the extraction is simple or surgical
  • The number of teeth removed
  • Grafting to preserve the socket, if chosen
  • Sedation, if used

Most dental plans contribute to extractions. Coverage for grafting varies. Our team can check your benefits and explain your options before treatment begins.

Frequently Asked Questions

Why would I need a surgical extraction instead of a simple one?

Surgical extraction is necessary when the tooth is not fully accessible, such as when it is broken below the gumline, impacted, or has roots that are curved, fragile, or fused to the bone. The approach allows the dental team to safely remove the tooth while protecting surrounding structures.

Is a surgical extraction more painful than a simple extraction?

The procedure itself is performed under anesthesia and is not painful. Post-operative discomfort may be slightly more than a simple extraction due to the additional tissue work involved, but it is manageable with prescribed or over-the-counter pain medication. Most patients recover comfortably within a few days.

How long does recovery take after a surgical extraction?

Initial recovery typically takes three to five days, during which swelling, mild discomfort, and dietary modifications are expected. The sutures may dissolve on their own or be removed at a follow-up visit. Full healing of the extraction site occurs over the following weeks.

Will I need stitches?

Yes. Surgical extractions involve an incision in the gum tissue, which is closed with sutures after the tooth is removed. Depending on the type of suture used, they may dissolve on their own or be removed at a brief follow-up appointment.

Can I get an implant after a surgical extraction?

In many cases, yes. Implant placement may be possible after the extraction site has healed, typically three to six months later. Ridge preservation performed at the time of extraction helps maintain the bone for optimal implant placement.
